Transcona

Discover Transcona

The town of Transcona offers an excellent opportunity to amble back through history to a simpler time. Much of the space in this town is dedicated to honoring its humble roots as a rail line on the Grand Trunk Railway. When this rail line was being built, its workers anticipated that they could be stationed here for quite a while. As a result, many of them began building homes and settling their families in the area. It wasn’t long until the town itself was born. Today, this history is richly celebrated all over town. It is glorified in murals in the downtown area and documented in the Transcona Museum. Now located in what was once the Bank of Toronto and the former Municipal Office, this is the site of a historic building. You can discover more than 40,000 artifacts in their collection, each of which contributes to the history of the area. Perhaps you know Transcona better as the “Park City.” It was unofficially dubbed the Park City due to its large green spaces and commitment to maintaining recreational areas. You could spend an afternoon at Kiwanis Park looking at the locally-made steam engine. Alternatively, you may take an amble down the Transcona Trail and stop for a picnic lunch. There’s no shortage of things to do and see in Transcona. However, individuals who are struggling with aging, a disability, recovery and rehabilitation following a surgery, or a complex medical condition may be faced with the possibility that they won’t be able to stay a part of the community. Partners for Home is a leading home care services company focused on enabling Manitoban’s the remain safely in their homes and united with their families. The services we offer can range from a few hours a week to full, 24×7 live in support. Our family created Partners for Home, after our father was diagnosed with ALS in 2007. Since this time we have become experts in helping families understand and navigate through the options here in Manitoba for the highest possible Home Care experience. Because of our how company works with the WRHA’s Self / Family Managed Home Care program, not only was our father able to remain in his home, but he continues to feel safe and remain a family contributor while maintaining his dignity. None of this would have been possible without this program. SFMC allows Manitoban’s through a private Agency to build a truly customized level of service specific to their individual needs. For our family, this meant our father could be helped out of bed at the time he wanted to be moved instead of waiting sometimes for hours for someone to arrive. Rather than having to deal with new people coming to our house almost every day, he could rely on the same Care Giver. This resulted in a less stressful environment for both our father and the Care Givers, as he no longer needs to retrain a new worker each and every day on how to move him. In addition, our father can have a shower as often as he wants, and is no longer limited to just one per week, which just was not enough. In addition, moving to the Self / Family Managed Home Care program, has enabled our family to develop meaningful relationships with the Care Givers that serve our family. We can help in almost every imaginable way from personal care activities to facilitating social support. It all starts with a simple meeting where we begin determining which of our qualified Care Partners is the best match for you and to build a truly customized personal home care plan that meets your individual and unique needs. Partners for Home makes every effort to maintain the same care partner to work with you reliably each day. This eliminates the need to train new care providers on a daily basis. Your staff will automatically know your routines and preferences when they arrive. Success between our caregivers and clients can frequently be influenced by the presence or absence of compatibility between the participants and we do not leave that factor to chance. Partners for Home leverages Insights Discovery, a world-class psychometric methodology which highlights an individual’s own strengths and weaknesses, communication style, approach to problems and value to a team. Insights Discovery is built to help people understand themselves, understand others and make the most of relationships that affect them.  This system has high value to our Clients and to all of our Home Care Partners.  Our Home Care services will help our Clients with Activities of Daily Living.  This may include showering, grooming, eating, and toileting – anything that you need to function on a daily basis. From here, they may move on to light housekeeping, including laundry, grocery shopping, or meal preparation. Throughout every task, your care provider should be focused on giving you as much independence, dignity, preference, and privacy as possible from the comfort of your own home. Depending on your needs, we can also help you to maintain social supports in and around Transcona. You can still upkeep your relationships with friends and family members by maximizing these opportunities to be a part of the community. Technology assistance is also available for the purpose of building relationships and staying connected. Family members can use this time to take a break from the hard work of caring for a loved one day in and day out. This can be an exhausting and often thankless task. While your loved one is in our care, you have an opportunity to take a break from this new routine you’ve developed and restore some balance to your life. Take this opportunity to keep your loved one home for longer. We would love to set up a no-cost and no-commitment meeting with you from the comfort of your own home to start discussing which services you would be eligible for. This is the first step toward creating a better future for you and your family.
